This section contains the document forms required to obtain the services provided by the Closed Joint-Stock Company Central Depository of the Kyrgyz Republic, as well as for carrying out depository operations and interacting with clients, issuers, and professional participants in the securities market.

The document forms have been developed in accordance with the legislation of the Kyrgyz Republic, the regulatory legal acts governing depository activities, and the internal documents of the Central Depository.

• Applications for opening, amending, and closing securities accounts (depository accounts);

• Client and depository system participant questionnaires;

• Instructions for depository operations;

• Powers of attorney and sample templates for their execution;

• Request forms for obtaining account statements, certificates, and other information;

All document forms are provided in their latest version and are available for download. Before submitting any documents, users are advised to review the instructions for completing the forms and the requirements regarding the completeness of the supporting documentation.

In the event of amendments to the legislation of the Kyrgyz Republic or to the internal regulatory documents of the Central Depository, the document forms will be updated accordingly and published on the official website in a timely manner.
